Lilly Cares Foundation

Humalog, Lyumjev, Basaglar

Provides free Lilly insulin to qualifying patients who are uninsured or underinsured. Covers most Lilly insulin products. Must be a US resident, not enrolled in Medicare Part D or Medicaid, and have a household income at or below 400% of the federal poverty level.

Novo Nordisk Patient Assistance Program

Novolog, Levemir, Tresiba, Fiasp

Free Novo Nordisk insulin for eligible uninsured or underinsured patients. Requires a valid prescription, US residency, no current Medicare/Medicaid coverage, and income at or below 400% FPL. Also offers the $35 insulin cap program for those with commercial insurance.

Medicare Extra Help (Low Income Subsidy)

Medicare Part D prescription costs

If you're on Medicare and have limited income, Extra Help can pay part or most of your prescription drug costs — including insulin, test strips, and diabetes supplies covered under Part D. You may qualify with income up to 150% FPL. Can save you up to $5,000 per year on prescriptions.
